NDC Creditors and parties in interest can obtain case information in active bankruptcy cases for a fee through the National Data Center.

Local and national forms such as Proof of Claim, Transfer of Claim and Change of Address are available on the Bankruptcy Court’s website. These forms change frequently, so check the court’s website to make sure you are using the current form.
